Request for Time Off Form

SAGE recognizes taking time off helps you stay balanced, refreshed, and honors the things in your life that are important to/for you. SAGE prides ourselves in working hard to approve all submitted requests for time off, within the policy and reason. SAGE values your continued hard work and dedication to our mission, values, and beliefs. Since we are essential workers, supporting individuals who may require up to 24/7 supports, approving all requests for time off may not be possible but SAGE will continue to do our very best to accommodate your request for time off. SAGE Policy(s):
1.) Standard (Non-SAGE Holiday) requests for time off (vacation) must be submitted correctly and on time, a minimum of 14 calendar days in advance.

2.) SAGE approved Holiday requests for time off must be must be submitted correctly and on time, a minimum of 30 calendar days in advance. Requests for time off for SAGE’s holiday schedule (July 4th, Thanksgiving, Christmas Day, and New Year’s Day) are only permitted for employees after their first 1 year of employment.

3.) It is considered job abandonment/voluntary resignation if an employee: starts a leave of absence or time off without following the procedures, does not return as scheduled without properly getting an extension approved, does not report to work as scheduled if the request for time off is denied.

SAGE considers the following factors when reviewing/approving requests for time off...
1.) Is the request within policy?
2.) Are the requests for time off considered excessive (jeopardizing the employee's full time status or stability/quality of supports provided to persons served)?
3.) In the employee's absence, can SAGE meet the needs of the person served? (Other coverage options, training new employees to cover, altering the schedule to still support the person and give the time off...)
4.) Which request was submitted first if multiple requests are from the same team of employees supporting someone? *SAGE grants requests for time off on a first come first serve basis, not seniority.

Employees are SOLELY responsible for verifying the request has been approved by checking their eRSP accounts. (Requests are approved and entered typically within 3 business days of receiving the request) If the request is either submitted incorrectly or entered incorrectly, the employee must communicate directly (in writing) to the strategy department immediately to correct the error. If the error is not communicated, the employee will be expected to work according to their schedule in eRSP.
